Blood flow restriction (BFR) training has become a popular alternate rehabilitation modality, intending to improve muscle strength, hypertrophy, metabolic response, and functional outcomes in diverse populations. However, there is a paucity of research evidence on the effectiveness of using BFR during aerobic exercise. Most BFR interventions have used BFR cuffs in two to four limbs, however, it is unknown if using BFR cuffs in a single limb instead of multiple limbs can produce the desired outcomes in muscle physiology and muscular adaptations. Therefore, we have divided the purpose of this study into the following three aims: Compare physical performance measures of balance, isokinetic muscle strength, knee stability, and aerobic capacity between cycling exercise with and without BFR. Compare physical performance measures between performing cycling exercise at 60% versus 80% blood flow occlusion. Compare physical performance measures between bilateral BFR versus single-leg BFR during a cycling protocol. These aims will be accomplished by randomizing 55 participants into five groups of eleven participants each: 1) control; 2) bilateral BFR cuffs with 60% occlusion; 3) bilateral BFR cuffs with 80% occlusion; 4) single-limb BFR with 60% occlusion; and 5) single-limb BFR with 80% occlusion. All participants will undergo balance, isokinetic knee strength, knee stability, and aerobic testing at baseline, 3-week, and 6-week follow-ups. The intervention for all groups will consist of 15 minutes cycling on a stationary ergometer at 70 revolutions/minute. A within- and between-group (5) by time (3) repeated measures analysis of variance (ANOVA) will be used to explore differences between group and time in addition to the interaction between these two.

Purpose: The objective of this proposal is to study the safety and efficacy of metformin added to insulin for treatment of type 2 diabetes mellitus (T2DM) among pregnant women. Participants: 950 pregnant women with type 2 diabetes complicating pregnancy from 10 U.S. clinical centers Procedures (methods): Pregnant women with T2DM between 10 weeks and 22 weeks 6 days and a singleton fetus will be randomized to double-blinded insulin/placebo versus insulin/metformin. Primary outcome is composite adverse neonatal outcome (clinically relevant hypoglycemia, birth trauma, hyperbilirubinemia, stillbirth/neonatal death). Study visits monthly at clinical visits; blood draw at 24-30 weeks, newborn anthropometric measurements at less than 72 hours of life. Maternal and infant outcomes will be chart abstracted.

Weight gain following antiretroviral therapy (ART) initiation occurs with all modern regimens. Recent real-world reports suggest that integrase strand transfer inhibitor (INSTI)-based ART may be associated with excess weight gain compared to other regimens. Weight gain appears to occur regardless of baseline weight, and is most pronounced among women and minorities, often those at highest risk of obesity-associated comorbidities. INSTI- and TAF-based regimens are now preferred regimens for most persons according to the Department of Health and Human Services ART-Treatment Guidelines. As a result, there is an urgent need to understand the underlying mechanisms for this weight gain. This study aims to understand the changes in energy balance that occur with changes in ART. Participants with HIV who have experienced \>10% weight gain on INSTI (bictegravir or dolutegravir-based therapy) will be switched to doravirine for 12 weeks, and then back to their prior INSTI regimen, allowing for assessment of changes in metabolic parameters with drug withdrawal and reintroduction (with no change to NRTI-backbone). Twenty-four hour energy balance will be measured on both regimens during a 24-hour stay using a whole room indirect calorimetry, with a standardized diet. Ultimately, the investigator's goal is to understand the mechanisms of weight gain so that future interventions can most effectively mitigate ART-associated weight changes.

RATIONALE: Drugs used in chemotherapy use different ways to stop cancer cells from dividing so they stop growing or die. Monoclonal antibodies such as gemtuzumab ozogamicin can locate cancer cells and either kill them or deliver cancer-killing substances to them without harming normal cells. Combining more than one drug or combining monoclonal antibody with chemotherapy may kill more cancer cells. It is not yet known which treatment regimen is more effective for acute myelogenous leukemia.
