The information collected will optimize the management of patients with urinary tract infections with secondary bacteremia, primarily with gram negative bacteria especially Escherichia coli. The current IDSA guideline endorses the use of fluoroquinolones and trimethoprim-sulfamethoxazole as the first line agents. However, over use of this empiric regimen could result in in appropriate treatment of bacteremia.

The proposed endpoints of the study would be: comparative use of piperacillin versus broader spectrum agents (e.g., piperacillin/tazobactam, etc.) \[measured as defined daily doses per 1000 patient days\]; physician acceptance of piperacillin as part of a streamlining program \[measured as successful occurrences of the use of piperacillin as streamlining therapy\]; changes in susceptibility patterns of broad spectrum antibiotics \[measured as % Gram negative bacilli susceptible to each of the commonly used broad spectrum antibiotics\]; and outcome of patients treated with streamlined therapy.

It has been demonstrated that panresistant strains of Acinetobacter species may be selected by antibiotic use \[4\], may be transmitted from person to person \[5\], and may be passed via environmental contamination \[6\]. Surveillance for panresistant Acinetobacter species should be a priority, given the lack of antibiotic options for the treatment of these infections. There are currently no data on the antibiotic susceptibility of Acinetobacter species or on the rates of panresistant organisms. The elucidation of potential risk factors for resistant strains of Acinetobacter is therefore an important task, and the use of alternative antibiotics should be considered in ICUs where these strains are endemic.
